Second Chance case manager no longer employed with Callaway County Special Services

CALLAWAY COUNTY, Mo. - ABC 17 News has confirmed a case manager responsible for Second Chance Homes of Fulton is no longer employed with Callaway County Special Services. 

According to a release given to ABC 17, Callaway County Special Services had issued "ongoing, multiple notices of non-compliance of Second Chance Homes." The release also said CCSS has "now completed an additional, internal investigation regarding this provider and reported our findings to both law enforcement and the Department of Mental Health." 

CCSS said they have been told not to release their findings while law enforcement investigations continue. 

CCSS also said "it is all but certain" when a case manager visited Second Chance monthly, reports indicated those visits involved face-to-face contact with residents. CCSS, however, believes the meetings were not face-to-face. 

Following their findings, the release said the case manager is no longer employed with CCSS.

CCSS also said it has verified the failure in oversight and it appears to be limited to one Department of Mental Health client. CCSS has refunded to Medicaid all payments made during the case manager's work with that client. 

It's not clear how much money was refunded to Medicaid but according to documents obtained by ABC 17, Second Chance Homes received an average of $561,000 from the Department of Mental Health over a nine year span, from 2009 to 2017. They also received an average of $31,000 a year over the same time span from "consumer funds" which could include Social Security money or other personal money used to pay for Second Chance services. 

CCSS said it has promoted a registered nurse to full time to be able to intervene in cases in which issues with providers or clients arise. 

The release also said CCSS is continuing visits, calls and contacts for each DMH client on its caseload.

CCSS also said it has given refresher training to existing staff members. The agency has also implemented a stronger structure to support multiple staff verification. 

Additional documents show multiple instances of Second Chance not filing paperwork on time, not taking daily log notes and canceling appointments with CCSS.
