Downtown Columbia bar cited for mold on beer lines

EDITOR'S NOTE: This story has been corrected to note the mold was on the outside of the beer lines, not inside the lines.

COLUMBIA, Mo. (KMIZ)

A downtown Columbia bar was cited for six violations, including having mold build-up on the outside of its beer lines.

Inspectors from the Columbia/Boone County Public Health Department inspected more than 150 eateries during a more-than-four-week span from Feb. 5 to March 10.

Inspectors visited Campus Bar and Grill on Ninth Street in downtown Columbia on Feb. 10. Inside, they found two critical violations and four non-critical violations.

Campus Bar and Grill: 2/10, all violations will need to be corrected by June.

  • Two critical violations: No hand soap at hand sink and no place to dry hands at hand sink. The inspection report said hand soap and paper towels were placed at hand sinks at the time of inspection.
  • Four non-critical violations: Cooler with beer has liquid build-up, a cooler door does not shut properly, the dining area has dust build-up and beer lines have mold build-up on them.

All eateries inspected:

Auntie Anne's: 2/4

  • One critical violation: Front and back reach-in coolers were not maintaining the proper temperature range.

Arby's: 2/6

  • One non-critical violation: Dust build-up on fans in a freezer.

Arena Liquor (Corporate Lake Drive): 2/14

  • One non-critical violation: Ice cream freezer has ice build-up. The inspection report said this violation will be corrected by the next routine inspection in August.

Battle High School Culinary Kitchen: 3/5

  • One critical violation: Mold growing in ice guard in ice machine.

Bonsai Sushi: 2/5

  • One non-critical violation: Women's restroom door is not self-closing.

Breaktime (East Stadium): 2/20, all violations corrected at the time of inspection

  • Two critical violations: No towels at hand sink and raw meat stored near ready to eat food

Breaktime (Grindstone): 2/10

  • Three non-critical violations: Seals on pizza cooler were in disrepair, cabinets in lobby area need cleaning and no thermometer in pizza or two-door cooler on cook line.

Breaktime (Smiley): 2/24, all critical violations corrected at the time of inspection

  • Two critical violations: Alcohol cans were left in handwashing sink and spray bottles near chemicals were not labeled.
  • One non-critical violation: Soda was leaking underneath soda storage

Burger King (Business Loop): 2/24

  • One non-critical violation: Cleaning bottles were dirty and overly stained

C & S Grocery Store: 3/9

  • Two non-critical violations: Bathroom door is not self-closing and no trash receptacle in a restroom. According to the inspection report, these two non-critical violations are repeat violations. The inspection report said if these are not corrected by the next routine inspection, "a $110 re-inspection fee will be assessed."

CAAC Battle High School Culinary: 3/5

  • One critical violation: Mold growing in ice guard in an ice machine.

Candlelight Lodge: 3/3

  • One critical violation: Cigarettes were on the floor.

Casey's Convenience Store (Paris Road): 2/12

  • One critical violation: No paper towels at hand sink.
  • Two non-critical violations: Employee items stored with food and light out above the pizza oven. The inspection report said the light will need to be replaced by June.

Casey's Convenience Store (Clark Lane): 2/11

  • One non-critical violation: Soda draining tray had mold growth. This violation was corrected at the time of inspection, according to the inspection report.

Casey's Convenience Store (Rangeline): 2/25

  • One non-critical violation: Ice build-up in a freezer. The inspection report said this violation will need to be corrected by June.

Central Bank of Boone County (East Broadway): 2/24

  • One critical violation: Bare hand contact with butter.

Chance Elementary: 3/2

  • One non-critical violation: Missing ceiling tiles in the dry storage room. The inspection report said if this violation is not corrected by the time of the next inspection, they will have to pay a $110 fine.

Chester Boren Middle School: 3/2

  • One critical violation: Hello Goodness reach-in cooler not maintaining the correct temperature.
  • Three non-critical violations: Missing ceiling tile in the dry storage room, grease trap was not contained and a wash sink faucet and the toilet were leaking. The inspection report said if ceiling tiles were not replaced by the time of the next inspection, they will be fined $110.

Chim's Thai Kitchen: 2/27

  • One critical violation: Raw foods were not separated during thawing.
  • One non-critical violation: Incorrect thawing methods.

China Chef: 2/18

  • Two critical violations: Employee did not wash their hands correctly and raw chicken was stored above sauce.
  • One non-critical violation: Buckets were stored on floor in walk-in cooler.

Columbia Public Schools Warehouse: 3/3

  • One non-critical violation: Employee food was sitting on chocolate milk in ready to eat cooler.

CORE: 2/18

  • One non-critical violation: Hand sink was used to fill a sanitizer bucket.

Cornerstone Group Home: 2/12

  • Two critical violations: Raw meat was stored on ready-to-eat foods and potentially hazardous foods were not labeled with an expiration date.
  • One non-critical violation: Kitchen cabinets were dirty.

Cougar Cafe: 2/24, all violations were corrected on a follow-up on 2/26

  • One critical violation: Mold on ice machine.

Crushed Red: 3/2, all violations were corrected at the time of inspection

  • Three critical violations: Turkey was cooled incorrectly, sanitizer bucket near a soda machine was not the correct temperature and cutting board was not smooth and easily cleanable.
  • One non-critical violation: Wet dishes were stacked together.

Curries: 2/28

  • One critical violation: Hand sink was used for purposes other than handwashing.

Denny's (Knipp Street): 2/20, all violations were corrected on a follow-up inspection on 2/24

  • One critical violation: Microwave above ice cream and bread was dirty.
  • Two non-critical violations: Bread freezer does not close properly and has ice build-up and gasket on the back door broken.

DQ Grill and Chill: 2/7

  • One critical violation: Unlabeled spray bottle next to a grill.
  • One non-critical violation: No covered trash receptacle in women's restroom.

Dunkin Donuts/Baskin Robbins (Elm Street): 2/19, all violations corrected on a follow-up on 2/25

  • Three critical violations: Sink out of order in women's restroom, no soap at the sink in the women's restroom and hand dryer was not working in the women's restroom.
  • One non-critical violation: Toilets in restroom backed up due to plumbing issues in the building.

Edible Arrangement: 2/25

  • One non-critical violation: Employees do not have food handler cards.

End of the Rainbow Southwest (Daycare): 3/6

  • Three non-critical violations: Unlabeled bottle containing oil, employee food stored above the establishment's foods and body spray was stored with foods in a cabinet.

Ernie's: 2/27

  • Two critical violations: Nesting containers were in the food and chemical bottles were not labeled.
  • One non-critical violation: Insulation not covered in the storage area.

Father Tolton High School: 3/4

  • One critical violation: Missing date label on coleslaw in walk-in cooler.
  • One non-critical violation: Employee coat was stored on shelving in dry storage.

Going Bonkers: 2/24

  • Two critical violations: An employee drink was sitting near clean dishes and raw chicken was sitting above raw hamburger in walk-in cooler.
  • One non-critical violation: Hot water at hand sink was not at the correct temperature.

Gumby's Pizza: 2/25, all violations were corrected at a follow-up on 2/28

  • One critical violation: Dish machine was not at the correct temperature.

H Town Market: 2/26

  • Two critical violations: Bare hand contact with ready-to-eat food and taco meat was not at the correct temperature.
  • Two non-critical violations: Lid on pizza cooler was broken and ice chute on soda fountain was unclean.

Harold's Doughnuts (Nifong Blvd): 2/20

  • One non-critical violation: Vents in walk-in cooler were dirty

HyVee Bakery/Deli (West Broadway): 2/12, all violations corrected on a follow-up on 2/19

  • One critical violation: Food not at the correct temperature.

Imo's Pizza (Business Loop 70)

  • One critical violation: No hand soap at handwashing sink.
  • One non-critical violation: Food stored on floor in walk-in cooler.

Italian Village (Vandiver Drive): 2/12

  • One critical violation: Missing test strips for bleach.
  • One non-critical violation: Missing thermometer in a cooler.

JJ's Cafe: 2/19, all critical violations corrected by a follow-up inspection on 2/21.

  • Four critical violations: Raw hamburger sitting above hashbrowns in a cooler, employee drink sitting on top of a food preparation surface, dish machine not reading for sanitizer (dishes were manually sanitized until the dishwasher was fixed), missing test strips to check sanitizer concentration.

Knights of Columbus: 2/28

  • One critical violation: Mold in an ice machine.

LA Nickels Golf Course: 2/21

  • One critical violation: Incorrect sanitizer concentration level.

Lee's Asian Market: 2/28, all critical violations corrected during a follow-up on the same day.

  • One critical violation: Cooler was not in the correct temperature range. All the potentially hazardous foods were moved to a new cooler.
  • One non-critical violation: Common name missing on frozen products.

Level Up: 3/5, The inspection note also said they discussed "making sure ice scoopers are not improperly stored in ice storage" and "discussed with manager that only the sale of cheese and vegetable pizza is acceptable."

  • Two critical violations: Improper sanitizing method (not sanitizing dishes) and missing test strips for sanitizer.

Mari's: 2/5, follow-up on 2/7 showed all critical violations were corrected.

  • Two critical violations: Dirty ice machine and walk-in cooler not maintaining the proper temperature.

McDonald's (East Business Loop 70): 2/7, all critical violations corrected during a follow-up inspection on 2/17.

  • Three critical violations: A handwashing sink is not working, sliced apples were not at the proper temperature and a cooler underneath the McCafe machine was not maintaining the proper temperature of 41 degrees or below.
  • One non-critical violation: Missing ceiling vent in the kitchen.

Midway of Columbia: 2/10, follow-up inspection on 2/12 said all critical violations were corrected.

  • Four critical violations: Improper method of dishwashing, improper timekeeping for the items inside of a warmer, handwashing sink used for purposes other than handwashing and clean utensils stored adjacent to handwashing sink.

Midway Truck Stop Convenience Store: 2/27

  • One critical violation: Packaged sandwiches under a heat lamp were not properly labeled.

Noodles and Company (Conley Road): 2/26

  • One non-critical violation: Dust build-up on the vent above the order counter. The inspection note said the vent will need to be cleaned by the next routine inspection which is set for June.

Panera Bread (Located inside the Columbia Mall): 2/26

  • Two critical violations: Milk and cream at the coffee station werr over 41 degrees, which is out of the proper temperature range and the dish machine's sanitizer was too low.
  • One non-critical violation: Dirty non-food contact surface.

Peggy Jean's Pies: 3/5

  • One critical violation: Dented cans. The inspection note said the dented can was removed from service at the time of the inspection.

Penn Station Subs: 2/26, follow-up on 2/28 said all critical violations were corrected.

  • Two critical violations: Unlabeled spray bottles throughout the establishment and a handwashing sink was out of order.

Phoenix Programs: 2/10

  • One critical violation: Dented can. The inspection report said the dented can was discarded at the time of the inspection.

Pickleman's Cafe (East Broadway): 2/28

  • One non-critical violation: A door does not shut properly, however, the temperature inside the cooler is within safe range. The inspection report said the cooler will need to be fixed by the next routine inspection in June.

Pizza Hut (Clark Lane): 2/17, all critical violations corrected by a follow-up inspection on 2/20.

  • Two critical violations: Medicine found next to food and milk, lettuce, tomatoes and raw chicken were above 41 degrees. The inspection note said all of the food was thrown away during the inspection.
  • Two non-critical violations: Missing scoop in the ice machine and cellphones, body lotion and body spray found next to food.

Pizza Hut (West Worley St.): 2/4, all critical violations corrected by a follow-up inspection on 2/7.

  • Two critical violations: Missing paper towels or dryer in the women's restroom and cheese and meat sauce in a cooler was at 60 degrees, which is out of the proper temperature range. The inspection report said all the potentially hazardous foods were thrown away.

Poppy's Place: 2/25, all critical violations corrected by a follow-up inspection on 2/28.

  • One critical violation: Missing sanitation test strips.
  • Two non-critical violations: Missing thermometer in a cooler and missing handwashing sign at the handwashing sink in the kitchen.

Prenger Foods: 3/3

  • Three non-critical violations: Packaged chicken sandwich not labeled properly, unshielded light bulbs in a walk-in dairy cooler and the cutting board used for meat cutting is no longer cleanable.

Prenger Bakery/Deli: 2/25, all critical violations corrected by a follow-up inspection on 2/28.

  • Two critical violations: Packed food items were not properly labeled and the deli display was not maintaining the proper temperature range.
  • One non-critical violation: Unshielded lights above the deli service line/kitchen.

Regal Columbia Stadium 14: 2/19

  • The inspection started due to a complaint about a sewage smell coming from theater 10. According to the inspection report, the manager was working on figuring out where it was originating. The inspection note said the inspector will check back during a routine inspection.

Residence Inn: 2/17, all critical violations corrected by a follow-up inspection on the same day.

  • One critical violation: Dishwasher not reading the proper sanitation level.

Rose Music Hall: 2/19

  • One critical violation: Unlabeled spray bottles

Ruby Tuesday: 2/24

  • One non-critical violation: Wet nesting dishes on drying rack.

Seasoning 63 (Old 63 South): 2/28

  • One critical violation: Dish machine not at the proper sanitation.
  • One non-critical violation: Walk-in freezer has debris accumulation on the floor and ice accumulation on the floor.

Shakespeare's Pizza (Peachtree Drive): 2/17

  • One critical violation: Dented can.
  • One non-critical violation: Fans near dish rack have heavy dust buildup.

Shepherds Basket: 2/24

  • One critical violation: Food not from an approved source. According to the inspection report, that food was thrown away at the time of the inspection.

Social Room: 2/27

  • One critical violation: No towels by the front hand sink.

Sonic (Paris): 2/13

  • One non-critical violation: Sink in disrepair.

Sonic (Centralia): 2/17

  • One non-critical violation: Walk-in freezer leaking condensate on shelves.

Sub Shop (8th Street): 2/20

  • Five non-critical violations: Lid on trashcan in restroom missing, packaged goods not labeled, coats and backpacks stored on flour in dry storage area, no one present with food manager training and employees need food handler cards.

Sub Shop (West Worley): 2/12, violation was corrected by a follow-up inspection on 2/19.

  • One non-critical violation: Plumbing issue causing drain to back up occasionally.

Subway (East St. Charles): 2/25

  • Two critical violations: Meatballs not cooked at the correct temperature and mold found in an ice chute on a soda machine. The inspection reports said all critical violations were corrected at the time of the inspection.
  • Two non-critical violations: Ceiling vents need cleaning and no food handler cards and no proof of manager certification.

Subway (East Business Loop 70): 2/19, all violations corrected by a follow-up inspection on 2/24.

  • Three critical violations: Walk-in cooler not at the correct temperatures, sinks have no handwashing soap and sink has no towels.
  • Two non-critical violations: Sinks do not have signage and employee bathroom door is not closing.

Subway (South Ninth St.): 3/10

  • One non-critical violation: Dishes were not completely dry before stacking them.

Taj Mahal: 2/27, all critical violations corrected by a follow-up inspection on 3/3.

  • Two critical violations: Raw meat stored over onions and dish machine is not working.
  • Five non-critical violations: Sink needs to be re-sealed and door on cooler needs to be repaired, single-use items stored where contamination is at risk, cutting boards need to be replaced, dirty microwave and utensils not stored properly.

The Deuce: 2/10, all violations were corrected by a follow-up on 2/13.

  • Two critical violations: No test strips to test sanitation concentration and bleach concentration in spray bottles too strong.

Tiger Bounce: 2/20, all critical violations were corrected by a follow-up date on 2/27

  • One critical violation: No hand sink in new cupcake bar area.
  • Four non-critical violations: Single-use utensils set out for self-service facing incorrectly, flooring in cupcake bar area not smooth and easily cleanable, missing lids for trash cans in the restrooms and tape used to repair a broken door handle.

Twain: Missouri Taproom: 2/13

  • One critical violation: Dishwashing sanitizer concentration low. This violation was corrected at the time of the inspection.

U Knead Sweets: 3/2

  • Two critical violations: Bread items stored in lunch bag and unlabeled spray bottle at drink topping station.
  • One non-critical violation: Single-service items stored in an area at risk for contamination.

UCP Heartland: 2/24

  • One critical violation: Food kept past seven days. The food was thrown away at the time of inspection, according to inspection records.

Wendys (Clark): 2/25, all critical violations were corrected by a follow-up on 2/28

  • Two critical violations: Mold on ice guard in ice machine and dirty microwave
  • One non-critical violation: Wet dishes stacked before completely drying.
